To help BushED manage reimbursement and support for families and volunteers, those of you making a claim are asked to complete this form.

You will need to submit a copy of the receipt where you have paid and are approved for a reimbursement or claim form for our volunteers claiming travel costs to support families.

Importantly you will need to submit a Claim Code which you will have been issued when you made an application. If you claim is for an approval prior to 6 Jan 2023 please use “Prior0623” as the claim code.

The documents I have supplied here are proof of expense related explicitly to the approved activity. I understand BushED will review and may ask for further verification before approval for payment is given. I acknowledge payment will be made into the bank account provided and any error by BushEd in processing the payment into this account if incorrect will not be the responsibility of BushED. No additional payments will be made in the case of an error being made where BushED has transferred to the nominated account.
